DAVAO CITY PUBLIC ADVISORY
The DQR registration for INDIVIDUALS resumed at 1:30 PM, Monday, November 16, 2020. The new link is:

Individuals who have already registered last October 31 to November 4 no longer need to register again.
1. The DQR link for INDIVIDUALS will be open to all who need to move around Davao City to work, do business, and access goods and essential services.
Please ensure to prepare the following before you go online and access the link:
A. a clear photo of your entire face, facing the camera;
B. a clear photo of your valid ID or birth certificate (a list of valid IDs is provided below);
C. a clear photo of your face beside your valid ID or birth certificate;
D. a valid mobile phone number;
E. a valid email address.
Employers should assist all their workers who do not know how to go online or do not own a smartphone.
2. Davao City residents who do not have a valid ID or birth certificate may apply for their DQR at the nearest police station.
3. Individuals 15 years old and below ARE NOT YET required to register with the DQR. The link restricts the registration of those belonging in this age range for now. Registration shall be announced on a later date.
4. Everyone entering Davao City will need a DQR. Non-Davao City residents who do not have a valid ID or birth certificate must first secure the requirements before they can register for a DQR.

a. Social Security System (SSS) / Government Service Insurance System (GSIS) Unified Multi-Purpose Identification (UMID) Card
b. Land Transportation Office (LTO) Driver’s License. Student Permit may be accepted if in card format.
c. Professional Regulatory Commission (PRC) ID
d. Overseas Workers Welfare Administration (OWWA) / Integrated
Department of Labor and Employment (iDOLE) card
e. Commission on Elections (COMELEC) Voter's ID or Voter's
